Include Proof of Age: Attach a copy of the Prospective Junior's birth certificate with the completed application.

Submit in Person: Bring the completed application to the Huntington Manor Fire Department at 1650 New York Ave, Huntington Station, NY 11746. Hand the application to the dispatcher, who will place it in the Juniors Mailbox.

After receiving your application, an advisor will contact you to schedule an interview with the Prospective Junior and a parent/guardian.
